Affinity Network
You have a lot to think about when choosing the right health benefits for you and your family. Our exclusive provider organization (EPO) plans give you access to quality medical services, including clinics, labs and hospitals. We’ll also help you connect with a primary care provider (PCP) and work closely with the rest of your care team. We’re here to support your health journey.

To help you manage your health, you will be required to select an in-network primary care group. By establishing a relationship with a primary care group, we can work together to achieve your health and wellness goals based on your history and preferences.
Our plans support your personal healthcare needs through partnerships with you, Moda Health, and your in-network providers. Once you’ve selected a primary care group, our plans use the Affinity Network to provide cost-effective, coordinated care to achieve your health and wellness goals.

Pharmacy benefits
The Navitus Network includes access to 87% of pharmacies in Oregon and 62,000 pharmacies nationwide. This means you can fill prescriptions almost anywhere, including these local and national drug store chains:- Costco

Travel with peace of mind
Our medical plans ensure you are covered when travelling anywhere in the United States and U.S. territories.
When you’re traveling outside of Oregon, you’ll get full-service medical care (starting Jan. 1, 2026) with in-network benefits through:
- Moda Select Network service areas in Alaska, Idaho and Texas
- Aetna® PPO Network through Aetna Signature Administrators® nationwide. This includes service areas outside the Moda Select Network in Alaska, Texas and outside the Affinity Network in Oregon.

Our network of dental care providers offers you the freedom to choose your own dentist, fast and accurate claims payment, and cost savings by staying in-network. Delta Dental of Oregon covers all of Oregon, and gives you access to providers nationwide.

With our individual and family dental coverage, you'll have access to the Delta Dental PPOTM Network and the Delta Dental Premier® Network. These are two of the largest preferred provider dental networks in Oregon and across the country.
The Delta Dental PPO Network offers a selection of more than 1,300 in-network dentists in Oregon and 114,000 nationwide. One of the largest dental networks in Oregon, the Delta Dental Premier Network gives you more choice, including 153,000 participating dentists nationwide.
Delta Dental Plan of Oregon also offers the Willamette Dental Network. With nearly 50 connected locations across Oregon, Washington, and Idaho, this network gives you seamless, personalized care no matter which Willamette Dental office you visit.

Medicaid redetermination is when the state checks to see if you still qualify for Medicaid. If you no longer qualify for Medicaid because of a redetermination, you may find affordable coverage through a subsidized ACA Marketplace plan.
